Each state shows the non-facility rate for one representative Medicare locality. Most states have only one locality, but several (CA, NY, TX, FL, IL, MA, MO, NJ, OR, WA, PA) have multiple. For those, we use the largest metropolitan locality. States like California span a wide GPCI range — billers in different California cities should check the locality-specific rate on the main code page.
