Methodology

Each state shows the non-facility rate for one representative Medicare locality. Most states have only one locality, but several (CA, NY, TX, FL, IL, MA, MO, NJ, OR, WA, PA) have multiple. For those, we use the largest metropolitan locality. States like California span a wide GPCI range — billers in different California cities should check the locality-specific rate on the main code page.

Rates derived from 2025 CMS Physician Fee Schedule (RVU25A) with 2025 GPCI adjustments. Conversion factor: 32.3465.
