Your self-pay estimate, explained
What to request when you are uninsured or choosing not to use insurance.
Ask for it in writing
For uninsured or self-pay care, you can generally request a good faith estimate. Scheduling at least three business days ahead also usually triggers this requirement.
Keep each provider’s estimate
Save the written estimate and compare each provider or facility’s charges with its own estimate.
Review a large difference promptly
A bill at least $400 over that provider’s estimate may qualify for the federal dispute process. CMS generally requires starting within 120 days of receiving the initial bill; other conditions apply.
This guide screens next steps. CMS determines whether a specific dispute qualifies.
